This Privacy Notice elucidates the protocols governing the handling of medical information by Wellness Centrale and its associated personnel. Please take a moment to review this Notice carefully.

This Notice details Wellness Centrale's practices and personnel responsibilities regarding your medical information. It covers the use and disclosure of such information for treatment, payment, healthcare operations, and other lawful activities. Your medical information includes data identifying you and relating to your physical, medical, or mental condition, as well as any medical care provided.


Additionally, this Notice outlines your rights regarding your medical information. You have the right to access and control it, including reviewing and obtaining copies of your records, requesting amendments, receiving an accounting of disclosures, specifying usage and sharing preferences, and addressing privacy concerns.   • Research: Your medical information may be shared with researchers under strict privacy protocols, ensuring your identity remains protected. 
  • Payment: We utilize your medical information for seamless payment processing related to your treatment and services at Wellness Centrale.
  • Healthcare Operations: Your medical information may be used and disclosed for necessary activities related to our operations. These uses or disclosures include activities aimed at maintaining the quality of care, compliance with regulations, administrative purposes, fulfilling contractual obligations, addressing grievances, or handling legal matters.
  • Individuals or Family Members: With your consent, we may disclose medical information to family members, caregivers, or another person you identify as being involved in your care. 
  • Emergencies: In emergencies, your medical information may be disclosed to relevant entities to inform them of your condition or status. If you wish to object to this disclosure, you can submit a written request. However, in circumstances where you are unavailable or unable to agree or object, or in certain emergencies, we will use our professional judgment to determine whether such disclosure is in your best interest. 
  • Legal Requirements: We disclose health information as mandated by Federal, State, or Local Law. 
  • Workers’ Compensation: Your medical records may be released for workers’ compensation claims following legal requirements. 
  • Public Health Activities:  We may disclose your medical information for public health activities including (1) preventing or controlling disease, injury, or disability, (2) reporting deaths, (3) reporting abuse or neglect of children, elders, and dependent adults, (4) reporting reactions to medications or problems with products, (5) notifying individuals of product recalls they may be using, (6) notifying individuals who may have been exposed to a disease or at risk of contracting or spreading a disease or condition. 
  • Health Oversight Activities: We may disclose medical information to health oversight agencies as authorized by Law.
  • Legal Proceedings: Your medical information may be disclosed in response to valid court orders, subpoenas, or other legal directives.
  • Disclosure to Law Enforcement:  We may release medical information to law enforcement, as authorized or required by Law for purposes including (1) identifying or locating a suspect, fugitive, material witness, or missing person, (2) providing information about a suspected victim of a crime under certain limited circumstances when obtaining the person’s agreement is not possible, (3) reporting a death suspected to result from criminal conduct, (4) reporting criminal conduct at Wellness Centrale. In case of a medical emergency, reporting a crime, the crime’s location, victims, or the identity, description, or location of the person responsible for the crime. 
  • Decedents: Medical information may be shared for legal purposes with coroners, medical examiners, or funeral directors.
  • Specialized Government Functions: Your medical information may be disclosed to authorized federal officials for specific government activities.
  • Inmate Information: Medical data may be shared with correctional institutions as required by law for inmate welfare, if applicable.
  • Threats to Health and Safety: In specific circumstances, medical information may be disclosed to avert serious threats to health or safety as required by law.
  • Marketing: We will not release your medical information for marketing purposes without your authorization.
  • Sale: We do not release or sell medical information for marketing purposes without your explicit authorization. 

YOUR RIGHTS REGARDING YOUR MEDICAL INFORMATION

  • You have the right to inspect and receive copies of your medical information, with certain exceptions.
  • If you believe your medical information is incorrect or incomplete, you may request Wellness Centrale to amend it and input the correct details. 
  • You have the right to receive a list of certain disclosures made of your medical information.
  • You may request restrictions on the medical information used or disclosed for treatment, payment, or healthcare operations, as well as limits on disclosure to family members or friends involved in your care.
  • You can request Wellness Centrale to communicate with you about your treatment in a specific manner.
  • You have the right to obtain a copy of this notice.

OTHER USES OF MEDICAL INFORMATION

Any other uses and disclosures of medical information not covered by this Notice or the laws that apply to such use will only be made with your written permission. If you grant Wellness Centrale permission about you, you have the right to revoke that permission in writing at any time. Upon revocation, Wellness Centrale will cease any further use or disclosure of your medical information for the purposes specified in your written authorization, except if Wellness Centrale has already acted in reliance on your permission. Please note that any disclosures already made with your permission cannot be undone, and Wellness Centrale is required to retain records of the care provided to you.
